Here are some of the more frequently asked questions about our Home Services by Nurse (YMMP) program.

There is no additional charge to the patient except for the cost of prescriptions and medications.

Currently, our LPNs are able to visit once a day. If your patient requires more frequent visits, please contact our pharmacy to see if we can come up with a solution.

In addition to administering oral medications, insulin, and patches, our Nurses are qualified to perform injections for other medications such as long-acting antipsychotics and anticoagulants.

Our LPNs for YMMP can only visit patients in Vancouver for the time being.

Healthcare providers, health units, or case managers can fill out our referral form and fax it to 604-630-1001 or email it to us at lisa.le@yyoung.com or rx@yyoung.com.

Our focus is to assist patients with medication administration and management. Our nurses do not assist with daily activities such as dressing, cooking, feeding, cleaning, lift and transportation, and withdrawing blood for lab tests.

In order to provide best safety practices and accurate records, medications must be filled, dispensed, and administered by our pharmacy.
