How much does it cost to participate in the yYoung Medication Management Program?
There is no additional charge to the patient except for the cost of prescriptions and medications.
How frequent are the LPN visits?
Currently, our nurses are able to visit once a day. If your patient requires more frequent visits, please contact our pharmacy to see if we can come up with a solution.
Can you inject medication?
In addition to administering oral medications, insulin, and patches, our Nurses are qualified to perform injections for other medications such as long-acting antipsychotics and anticoagulants.
What areas do you serve?
Our LPNs for YMMP can visit patients in Vancouver for the time being.
How can I refer a patient to participate in the yYoung Medication Management Program?
Healthcare providers, health units, or case managers can fill out our referral form and fax it to 604-630-1001 or email it to us at rx@yyoung.com.
Do you provide other services in the yYoung Medication Management Program?
Our focus is to assist patients with medication administration and management. Our nurses do not assist with daily activities such as dressing, cooking, feeding, cleaning, lift and transportation, and withdrawing blood for lab tests.
Can you administer medication from another pharmacy?
In order to provide best safety practices and accurate records, medications must be filled, dispensed, and administered by our pharmacy.
